Yes, seriously, tactical pooping training is needed. I would guess that proper holsters, combined with re-buckling the belt, would help? And how to deal with dirty bathrooms when you can't wait for the janitor.... Sorry for the uncouth assignment.
John Lovell recently posted about using a public toilet while carrying. His solution was to remove the gun, holster and all, and stick it in the support rail. This keeps it easy to get to if needed while not having to worry about it falling out of the holster (should not happen with proper retention).

Many firearms have been “left behind” in public restrooms by doing things like this.
We all need to give this our full attention and perform readiness test of all security plan. Might be a good time to do a table top or live exercise
